Receiving the password notification by proxy

A proxy is allowed to receive the password notification under certain conditions.

Please refer to "Receiving the password notification by proxy at the Kashiwa Library" if you prefer to receive the notification at the Kashiwa Library.


Receiving by proxy

Conditions

If a user (hereafter referred to as "applicant") is unable to receive the "password notification" from the Help Desks/Service Counter of Information Technology Center for unavoidable reasons, the applicant is able to ask a member of the UTokyo (proxy) to receive the applicant's password notification (receiving by proxy).

What to bring to the Help Desk/Service Counter

1. A hand-written request completed by the applicant using the specified form (PDF form 1)

3. Proxy's ID

Procedure at the Help Desk/Service Counter

The proxy must submit 1 and 2 stated above and show 3 at the Help Desk/Service Counter of ITC. ITC will make a copy of 3 and the proxy will receive applicant's password notification on the spot.


The above procedure is the basic rule for receiving password notification by proxy. However the following method is also acceptable if the above procedure cannot be followed.

Conditions

1. The applicant must be a graduate student or graduate research student.

2. The proxy must be an academic supervisor.

3. Possible to receive both

4. The academic supervisor must belong to the faculty or graduate school and have an internal mail address.

What to submit via internal mail

1. A hand-written request completed by the applicant using the specified form(PDF form 2)

2. A copy of the applicant's ID


Receiving by proxy at the Kashiwa Library

If the applicant prefers to receive the notification using a proxy at the Kashiwa Library (the Service Counter is on the 2nd floor), you must contact ITC in advance. ""The proxy must submit"" the following documents to ecc-support [at]ecc.u-tokyo.ac.jp (replace [at] with @ for real email address) along with the request form.

What to send with the email

If you are unable to attach files to the email, fax (03-5841-0311) them to the person in charge of ECCS and send an email saying that you have a problem with attaching files.

Procedure after sending email

After you receive a notification from ITC regarding their completing the confirmation of the submitted documents, being the originals of the documents to the Service Counter at the Kashiwa Library. The subsequent procedures are the same as "Receiving by proxy" at the Help Desks of ITC.
